Overview

IGF-1 (insulin-like growth factor 1) is an endogenous polypeptide hormone critical for growth, development, and metabolic homeostasis. It is produced primarily in the liver in response to GH signaling and acts through IGF-1 receptors on virtually all tissues. Recombinant IGF-1 (mecasermin) is FDA-approved for children with severe primary IGF-1 deficiency.

Mechanism of Action

Activates IGF-1 receptors (IGF1R), a receptor tyrosine kinase, triggering PI3K/Akt/mTOR and MAPK signaling cascades that promote cell growth, protein synthesis, glucose uptake, and inhibit apoptosis. Mediates many of the anabolic and growth-promoting effects of growth hormone.

Safety Notes

FDA-approved (Increlex/mecasermin) for severe primary IGF-1 deficiency in pediatric patients. Risks include hypoglycemia, intracranial hypertension, and potential growth-promoting effects on existing neoplasms. Off-label use not FDA-sanctioned.
